Preferred Provider Signup - Description of our different Plans


The difference between an Exclusive and a Non-Exclusive plan is that with an Exclusive plan, you are the ONLY allowed provider in that county.  In addition, you have the opportunity to also include your advertisement graphic banner along with your advertisement information.

If you choose the Exclusive plan, please have your Ad Banner already developed and ready to upload in Step Number 2 of 2 in this signup process.
